Implement flagging delivery episodes in maternity activity #15

following discussion at the analytical steering group, this is to be implemeneted by introducing a flag that checks to see if the first episode in a spell had maternity_episode_type == 1. This needs to have a bit of a sense check to make sure it gives reasonably results (see https://github.com/The-Strategy-Unit/nhp_planning/issues/48)

This should probably be moved to nhp_data (leaving here for now until we have QA'd that process)
