Add additional columns to selected evidence table on summary tab #3

Allow user to select columns to view this table by/deduplicate by, such as country, setting, etc

The functionality is in the app, just need to make sure the selections are appropriate with the 'actual' data.

In the first instance, can remove demographics and study design.
