Updating 'Adjusting for health status' #125

Adjusting for health status
Currently reads "The scale and direction of this adjustment is supplied by the model user." I think this sentence should be removed as the adjustment is fixed.

There will also need to be a change made to Application in model "g. User supplies a health status adjustment parameter" & potentially "i. The model calculates a health status adjustment multiplier." although i. still maybe true if there is random sampling from a distribution of possible change factors determined by the expert elicitation???
