I installed the add on and the QR code does not show up when the Ankimote button is pressed

Can you provide the following:

Anki version
Operating system
Does the QR still not show up if all other addons are disabled?
Does the QR not show up if you press the ankimote button on the decks screen? How about if you press it on the reviewer screen with a flashcard shown?
