Add death sequence #105

When the player dies he just lies there waiting to respawn. It would be neat if 
the screen would fade to black and the text "you got killed" with "press a 
button to continue" shows. Add the possibility for level designers to specify a 
music .wav file to play during this death sequence as well.

When dying, the screen now fades to black and shows a "you died" and "press any 
button to resurrect" text. Still need to implement the death music.

Music is implemented as well now. Set a "deathmusic" key to worldspawn.
