[SwearIMG](or shorter equivalent) would operate as an exact double of the IMG tag (but it wouldn't be visible as a button above the post thingamajigger) but the functionality would be through the swear filter so you would add a swear filter so like

[SwearIMG] = [spoiler=NSFW, Swears][img]
[/SwearIMG] = [/img][/spoiler]

So that way you would be able to post an image with swears without it being in a spoiler tag but anyone who has the swear filter on WOULD have it in the spoiler tag anyways.

what if someone has the swear filter off because its much harder for someone glancing at their screen to see "@!$%" in text rather than a huge bold "@!$%" in a picture

Moderators spoilering the images that are NSFW due to swears is just fine as is. Nothing of value is lost, you can still view the images with a button press.
