Already a ZP member option

ZetaPrints UserID is managed by OpenX and stored in OpenX db per user.

When the option is checked on, remove any existing UserID from OpenX DB.

When the option is checked off, create a new GUID and store it in OpenX DB.
The GUID will be the new UserID.

Issue a warning about the change, but do not require confirmation.

Off -> On: "You may need to log in when you are redirected to the image
generator (ZetaPrints)"

On -> Off: "Your ZetaPrints account information will be removed from OpenX
and you will access the image generator as a new unregistered user."
