Preview gen. form #11

Hide Save button until there is something to save.

Replace "Please choose the type of the banner" label with "Banner type".

Put a line or some other separation between the banner properties (name,
type) and the preview form. Maybe put the properties into a fieldset with
label "Banner properties"?

This one wasn't done at all:

Now the name and type fields are part of the form. I can't tell where they 
belong.

Is banner name required or not?
