Bad feeds must eventually expire #30

Remove feeds which never load after a given grace period. Should perhaps notify 
subscribers by email?

To do this we need to know when they were added. Otherwise there is no way of 
knowing if it just failed once, or if it's been failing for weeks.

Added the column now, so at least we know when feeds were added. Next step is 
to purge the feeds and notify subscribers that they were purged.
