Activate Apache Rewrite (from Brady Miller) #5

  1. need to turn on apache rewrite (note the the modules doesn't work since Zend (and the new patient portal) require this. here's how i did it on the demo farm docker: https://github.com/openemr/demo_farm_openemr/blob/master/docker/pre-openemr/16-04/Dockerfile#L52
