admin adding off-site grade #12

After exempting a grade via a tutor's request, and re-issue it with a fail, I'm unable to proceed beyond selecting a procedure - which only has 1 item "non-procedural" for selection. Expected "procedural" or "non-procedural" as the selection, and to progress to grading, comment and save.

I've updated the manual procedure so it'll allow you to select any procedure. I've also added a section for you to write comments.

It is currently available on the test.uwa.dental site, I'll push it to the main site at night
