Rewrite extension loading to be more flexible.
Unify custom template functionality into a new custom_templates option.
Add clear_all_scopes sub-option to fix #59 while allowing #55 to still work.
Deprecate custom_template_tags and styled_components options in favor of custom_templates.

Relies on yaml-macros-engine v2.1.0 (released May 31, 2019).
