geoCodeType: codelist column names #348

The codelists generally have the column headers: code | title | description ...apart from geoCodeType.csv which is code | name | description | .... Ideally to be able to consume these more easily without having to program special cases for geoCodeType.csv it would have the same column names as the other lists.

I think this should be fixed and is a PATCH scope bugfix. It doesn't change the semantics of any codelists and brings the column headers in line.
