Sponsor Us modal broken on mobile #27

The Sponsor Us button on the very bottom is broken. Also the select your sponsorship type field is not showing up correctly

Default section not working. Not sure why, as I put "selected" on the option. Everything else is fine; so I'll leave it at that.
