reference-style rendering

Not sure that i really like the way you implement the rendering of the reference-style links.

Ordinary links are rendered as (clickable) links. Image links are rendered as (clickable) links AND the link target is still visible after the link. I think this is a nice way of doing this without hiding information.

For reference-style links

• the target url is not visible
• clicking results in a strange error message (e.g. cannot open 13)

Suggestions - use the same way as for image links:

• add quotes to the error msg to visualize spaces if there are any (e.g. cannot open '13')
• i think having a real translation of the references to urls is not really needed (will be great of course) but visualizing in the same way as for ordinary links might be good - simply have the [13] after the link just to visualize what happend's.

And the list of the references (in my case at the end of the document) is still uggly and misses the first entry

The fact, [2]: is not recognized as a link definition, is a bug causing the click on the ref not being found. I need to fix this...
