What to do when... chapters and sections incorrect? #16

What would you like to see clarified, corrected, or expanded upon?
It does not appear the referenced Chapter and Section numbers are correct, or 
even exist. For example, 1.16 Identifying Fundamental Problems references 
15.1.6 (doesn't exist), and there is no correlation between the chapters, 
sections, and topics.

My guess is that the sections and chapters were incorrectly updated between 
First and Second editions.
