So I went with theory 1 that the disk with XTF-BOOT is not the USA release, so added Mirage Technologies to help distinguish that release from the other.

World War II- Battles of the South Pacific (1993)(Quantum Quality Productions) [IMA][1440k].zip - this is tgod002951, likely US release based on MSDOS5.0 OEM ID. Minor difference in install.dat file.
