Step #2 - I don't see for which download the application charge MyRC$4. You
should see it if you scroll down. Check that and post the screenshot.

I'm not able to duplicate the issue. I installed a fresh Roundcube and an
initial download of Plugin Manger and its helper plugins
(https://myroundcube.com/myroundcube-plugins/plugin-manager).
Both the checkbox on top and the link on bottom "Discard incurring costs
downloads" rest MyRC$ to zero and gives access to free downloads.

I missed your post #6. Check javascript console. Javascript errors caused by
your Ad block should be the reason why the GUI does not reset as intended.

The adblock rule ##[name*="google_ads"] hides that cell.
Disabled it, now everyting is fine.
