add CPT IDs and labels #124

We plan to add a resource (Service Provider) that will connect indications (diseases) and procedures. The procedures will be CPT codes. We'd like them to be resolvable by this resource, meaning at minimum the label would be retrieved when the ID is sent.

Closing this issue, we're not going forward on adding this service because of the licensing issues
