List available orgs on member dashboard

As a member, I want to see a list of available orgs that I can share my data with, so I can give them permission to view my data and help me.

• On the /member page (dashboard), there's an organizations panel that should list approved orgs, and an edit link to the /member/id/organizations page.
• The /member/id/organizations page lists "Current Organizations", "Requests Waiting", and "Other Organizations".

• clicking on a "current" org pops up a verification modal

  • choices: "Yes, Revoke" or "Cancel"
• clicking on a "requests waiting" org pops up a verification modal
  • title: "Approve Access"
  • choices: "Approve Access", "Deny Access", or "Cancel"
• clicking on an "other" org (not current, not requested) pops up a verification modal
  • title: "Grant Access"
  • choices: "Grant Access", "Cancel"

When a state change is confirmed, the request is posted and the page is reloaded (no ajax for simplicity)

For now, orgs in smh_app are not connected with orgs in vmi.

For right now, you can just list all organizations right on the dashboard. Skip the search function for now.
