Provide the ability to connect this application to the alpha.sharemy.health data source for the logged in user.

[x] A view for connecting data sources
- [x] A button (link) for initiating the flow.
[x] A callback view for recieving the oauth "code" and exchanging it for a token
- [x] Store the token against the logged in user.
[x] "Disconnect" from smh button that revokes and removes token

Some future work may exist around token management, refresh, etc.
