implement Kiwi's redesign w/ small twists #14

Design is going up for consideration in the community, but wanted to get something up for now (draft PR).

PDF version can be found right here.

TODO

[x] Implement redesign for refreshed combo chart.
[x] Enable PDF generation (feature was missing).
[x] Add custom trippy hippie-dippy QR code for left side.
[x] Make sure everything actually renders in the output.
[x] Change left QR to hit https://combo.tripsit.me.
[x] Change "Benzodiazepines" to read "Benzos" so it fits.
[x] Add link to the Android app on https://combo.tripsit.me.
[x] Match QR code styling on right side if desired.
[x] Determine what localized verbiage needs to change.
[x] Experiment with left-aligned text (swap legend and disclaimer).
