Fix up pg_prefaulter to cooperate better on Illumos. Notable improvements:

Use /proc where possible to extract process arguments
Use pg_xlogdump(1)'s -f flag to stream files.
Found a bug in the purge invariant when rapidly starting/stopping PostgreSQL under load.
