Mention Source Code file wherever "Please refer source code" is mentioned. #215

When reading the paper on the unchained index, any mention of "Please refer to the source code" can be accompanied with the source code file + the line number of the code block being suggested as the reference.

Thanks. This is a good idea. We're in the middle of re-writing the spec for version 2.0.0 now, so I'll try to make sure this gets done.
