Form fields repeated for several things #13

Not sure why but generated form fields on views are repeated in some cases.

Found issue was due to way fields are being initially mapped. Creating mapping class and re-factoring core code to solve issue and allow more flexibility.

This issue has now been solved with latest bug fixes
