How does the assistant sign? #15

We have guidance from the SBE (#6) about the language to use for the voter to "sign" their request. But we have no language for the signature field for the "Assistant’s Statement/Info" section. How do we handle this?

Question about both signatures: do we have to include the actual signature field, for the user to fill in their name? The instructions say if an assistant is filling out the form, they have to write "Applicant Unable to Sign," but for the assistant, can checking the box constitute the signature? (The checkboxes are what I'm using to determine the boolean field in the JSON anyway.)
