ID all language that must be present under law #5

Beyond the text of the form itself. What disclaimers are required?

I think that the only text that's required is the text that appears before the signature checkbox:

I swear/affirm, under felony penalty for making willfully false material statements, that by pressing the Submit button and submitting this information that (1) the information I have provided on this form is true, (2) I am not requesting a ballot or voting in any other jurisdiction in the U.S., except the jurisdiction to which this application relates, and (3) that I am the person named in this absentee application.

I'm not sure about what text needs to appear prior to the "Assistant’s Statement/Info" signature checkbox. Of course, we can't use the same language as for the applicant signature, because that text doesn't apply. But I don't think it's as simple as just changing it—we only have that language because the VA State Board of Elections has specifically authorized it.
