Make registration place a pulldown? #8

In the PDF form, the applicant checks off "county" or "city" and fills in the blank:

locality

This is unnecessary for an HTML form. Just make this a pulldown where they select their locality?

That will make it possible to embed the GNIS ID, too. (That's the federal standard for IDs identifying places.) We use GNIS IDs as the key for registrars, with each GNIS ID corresponding to the specified locality.
