Although incapacitation by the infected causes the transition to SUSTAIN_PEAK (by maxing out the survivor intensity), a fast or instant death (by a Witch in Expert, for example) does not, which is not logical. I haven't tested if the survivor intensity is actually maxed out by death but isn't checked by the Director because it all happens in 1 tick, or if the intensity isn't updated at all. Regardless, even if reading the survivor intensity isn't reliable by design, there should at least be a hardcoded action/check to force the transition in such death cases.
