7 years ago I will get you a list of mods. Seems more prevalent in an SMP environment. Will let you know here shortly.
Having other weird problems as well and I cant seem to nail down to a single mod.

6 years ago I just released a new version (1.10.2 and 1.12+) that fixed some issues which may be related to this. Unsure if fixed since I never got to reproduce it. If you test it, please let me know.

6 years ago I am terribly sorry for not digging further. I've had a flurry of things happen that prevented me from even thinking about MC. I will check out the new version in the next day or so.

6 years ago I think this might be happening still on 1.11.2. It's not happening on 1.12.2 (to my knowledge, as my 1.12.2 pack has this mod in it), but I'm hosting a server for a friend that uses this mod in a pack, and this issue keeps happening for them.
Sorry but there appears to be another problem with my friend and I going through a portal. We've rebuilt the portal twice, but continue to die after going through.
And that is just me. I have to stop the server in order to get out of the loop. When it is restarted, I spawn back at some random coords in the ground and suffocate.
