Unable to add test name #11

Hi, Login credentials : http://182.50.154.124/health4all_v2_test User : admin/ abc123

Observation : When tried to test Services->Diagnostics->Add->Add test name module, I was unable to add a new test name, as it displayed a PHP and database error. Please find below snapshots for the process that was followed. testname1 testname2
