Services->Sanitation throws database error #13

Hi, Login credentials : http://182.50.154.124/health4all_v2_test User : admin/ abc123

Observation : When I tested the Services->Sanitation module, I filled up all the fields and clicked on 'Select' button. It immediately displayed a database error. Please find below attached snapshots for the procedure followed. services-sanitation

Hi, I tried to retest the above, and now it is not displaying database error, but there is a blank evaluation form being displayed. Please check the below snapshots.
