Unable to add hospital #16

Hi, Login credentials : http://182.50.154.124/health4all_v2_test User : admin/ abc123

Observation : When tried to test the Services->Sanitation->Add->Add hospital, I am not able to add a new hospital. When I try to select facility type or village/ town from the drop down, I am not able to do so, also if I fill up other required details, I find a database error after clicking the 'Submit' button. Please find below attached snapshots for the procedure followed.
