Unable to add department #17

Hi, Login credentials : http://182.50.154.124/health4all_v2_test User : admin/ abc123

Observation : When I tried to test the Services->Sanitation->Add->Department module, I am not able to add a new department; as it throws a database error after clicking the 'Submit' button. Please find below attached snapshots for the procedure followed. add dept
