1. Form Label - Add Sanitation Contract
2. Label<Facility:> Input Field\ - placeholder <!Display Facilities in Alpha sorted List of Values. On selection of facility, display hospital_type, village_town, district information>
   Label<Vendor:> Input Field\ - placeholder
   <!Display Vendors in Alpha sorted List of Values. On selection of Vendor, display vendor_city, contact_person, vendor_phone, vendor_email>
3. Label<From Date:> Input Field\ - placeholder
4. Label<To Date:> Input Field\ - placeholder
5. Label<Status:> Input Field\ ,<!Default value 'Active' , List of Values to include Active, Ended>

On submit, show on top 'Contract added successfully' and show below the form to add new contract
