Investigate delay when combining pyIGTLink and PLUS

Combining the data via PLUS is causing delays in two ways:

• The US image, while having a good framerate, has a lag of ~1sec.
• The resulting rendering in Slicer is slow compared to sending the tracking data through PLUS and the US from pyIGTLink directly.

This might be down to the PLUS configuration (e.g. the StartupDelaySec setting?), but we should investigate, because sending everything through PLUS would be a big simplification - for example, it saves us reimplementing the volume reconstruction (#41). If we cannot solve this, we should adjust by having two connections and doing the reconstruction in Slicer.

Following my revelation about the faulty power supply last week, this problem seems to have gone away. We can double check everything is OK, but I think we can close this issue.
