• Calibration error
• Updated transforms (after calibration) not being used (perhaps because they're being overwritten by original values)
• Reconstruction algorithm parameters requiring tuning
• Reconstruction volume being created with respect to the wrong transform

From experiments today, it seems that the image calibration is one source of error, at least.
