Add a `config.py` file #60

This should contain things like:

[x] PLUS IP and port
[x] Path to PLUS executable, Slicer should add it to the env PATH at every start-up
[x] pyIGTLink IP and port
[x] IGTLink connector name
[x] Any hardcoded strings, like transform names, CT name, US name, output directory
