Add comments box for neurostim info

A button will open up a text edit box, which will let users record:

• the input (in mA?)
• the output (whether or not there was a response, True or False - exact amplitute not required)
• any other comments

When closed it will save the neurostim location and the timestamp, along with the above. Maybe it makes sense at that point to also show the location in the viewer(s) (#66).
