[ ] Define the A3 model with all necessary properties
[ ] Update all mappers and form fields
[ ] Validate fields with Regex, range, custom validation, etc.
[ ] Add form to form contract
[ ] Update A3 UI to match NACC documentation.
[ ] Modify enable/disable behavior and implement on UI
[ ] Insure form matches NACC documentation with correct UI, logic, validation, behavior and ensure data is saving correctly

Hey @Oddvocado, noticed I overlooked the js is in the wrong place, could you fix this along with your current work?

Custom js should be moved to UDS.Net.Forms project in wwwroot/js folders so that it will be bundled with the nuget package (the path to the file will need to be updated to ~/_content/UDS.Net.Forms/js/A3.js) as well.
