[ ] update the C2 model with all necessary properties from C2T
[ ] Update all mappers and form fields
[ ] Validate fields with Regex, range, custom validation, etc.
[ ] Add form to form contract
[ ] Update C2 UI to match NACC documentation (when telephone modality for remote is used follow C2T, when in-person or video modality is used follow C2)
[ ] Modify enable/disable behavior and implement on UI
[ ] Insure form matches NACC documentation with correct UI, logic, validation, behavior and ensure data is saving correctly

In the test data submission, it looks as though the C2 and C2T are combined as one form:

So the view for C2T should probably only be modification of the display of some fields, and some custom validation would need to be used according to which modality.

If the visit mode is:

In-person, then the C2 fields should be completed
Remote, then the modality chooses which

Modality is:

Video, then C2 fields
Telephone, then C2T fields
