Add UI logic to only allow specific visit types for the selected participation.

Some rules to get started

[x] If they already have had an IVP or TIP then they cannot have another
[ ] If they have not had any UDS 3 IVP/TIP visits then they can choose IVP/TIP if the visit number is 1 (if they had a UDS2 IVP then they may start UDS3 visits with a visit number > 1)
[ ] If they do not have a milestone recorded for future telephone visits then they cannot create a telephone visit
[ ] If they have a milestone recorded for death or withdrawal then they can only create visits dates before the recorded milestone date

Let's review the rules in further detail when this card is started.

This should be implemented locally to take into account previous visits from earlier UDS versions. Each organization will need to implement this behavior themselves according to their own business rules.
