There are multiple ways the phantom item bug is triggered.
Not all are documented here.
@jaedan mentioned lot of logic issues in the Razor codebase in various areas that could be triggering these.
Below are some examples of reproducing:
1) Move an item to any location Razor is "blind" to... (existing stack, tome, unopened container)
2) Perform another action twice... (Interrupt twice, Dress twice / 2+ items, Organize/Restock twice / 2+ items)
3) Phantom item reappears
Does not happen when moving item to somewhere that Razor can "see" where the original item serial went or if the following action is not run twice.
Another Example:
When using bailout/cancel target script I can usually get items to reappear that have been moved by the organizer/restock agents
1) organizer agent moves stuff
2) hit bailout hotkey that has these 3 hotkeys in them
3) phantom items reappear
doesnt happen all the time though
hotkey 'Clear Scavenger Cache'
hotkey 'Clear Drag/Drop Queue'
hotkey 'Drop What You are Currently Holding'
