Resubmission Request form for ECMPS

Add a Resubmission Request form for ECMPS. The template/historic version is here: https://ecmps.camdsupport.com/help_resubmit_form.shtml

Only show the form for logged in users. Use CDX to get the personal info needed to complete the request, e.g., the form should not include requests for the user's name or email address.

The request itself should not be an official submission.

The submit agents for the data and the requestor should get an email when the request has been completed saying that the data can now be resubmitted.

@annalbrecht EPA would like to review the requirements of this ticket to provide a leaner solution. Could we discuss this further at the next meeting?

Potential new requirements

1. Provide this form to logged in users.
2. Remove the collection of PII, leverage CDX for contact info.

Update: Discussed with EPA on 6/7/24. Additional info added to the initial ticket description.

Update 8/29 - based on internal EPA discussion, moving forward:

• EPA will develop host this form on the EPA website
• Will remove the collection of PII
• ERG development no longer needed

Form has been developed on EPA website and ready to publish upon ECMPS 2.0 final launch.
