CAMS-283: update model #913

Purpose

We want to update the data modeling to enable other goals for CAMS-283.

Major Changes

add roles to CamsUserReference
add types to properly model USTP Office to court relationship
- add data for Seattle and New York offices
add Cosmos DB container definition for offices

Testing/Validation

Automated tests pass.
