Extract data from COP19 FAST into a dataset

Request to extract data elements and values from in-process COP18 FAST tools to help respond to analytic questions developed by the ER team.

• [x] develop package to read in "2 Intervention-E" and "3 Initiative E" tabs and make tidy. Most up to date fast tools (with no duplicates) are located here. ER team will maintain this folder

• [x] Modify/adapt the above to read in data from "5 Commodities E" tab. The ER team wanted the "Commodity Major Category" and "Commodity Minor Category" fields and their associated values included. The do not need the associated regimes, so the values can be summarized/collapsed up to the lowest level of minor category.

• [x] Import "6 CODB-P" tab.

• [ ] Modify an OU x IM MSD to include values for selected indicators for APR18 results and targets, and FY19 targets, and (eventually) COP19 targets.

Currently can read in all sheets from FAST except COBD tab.

run_fastR() now imports all sheets from the FAST. Output for each tab can be made into separate files with all countries included.

I successfully ran this code and have extracted all the tabs. I will post them in the ER team folder on Monday when I have permission for that folder. Resolves #102

Josh, can you re run in am? I'm pushing a new version that tries to (1) make the sheet outputs more compatible with one another and (2) creates an additional function that combines Interventions, Initiatives, and CODB tabs into one file.
