Fixed more sources of audit user/note errors. #712

I located some more locations where the audit user and audit note parameters were not being passed to postgresql by the Tuba code. I modified a number of files to address the problem. There may be yet others, but I was unable to find them. Most of these relate to deleting entries. I also attempted to address a situation where the error arose during file upload.
