Test using a google form to collect email addresses

Important thing to check out: when I have sent it to a few non-DOI addresses directly, via email, it asks me to confirm that I want to send it outside DOI. Need to be sure that a non-DOI person can follow a link to the form without being blocked.

LCC group will manage this - will provide a link eventually
