Minor updates as documented in RFC-0763.
Added four new attributes and fixed some issues with the scripted attributes.
Modified the nudge screen code and config to disable it without removing it.
Note that conf/authn/mfa-authn-config.xml has many whitespace-only changes (I indented a lot of code) but few actual changes.
