Looks like it happens because it's synchronized with the heartbeat. Can be pretty unfair if it aligns in a bad way.

There is nothing wrong with this video. That’s a normal attack and you dodge it perfectly.

I was wrong about it being unfair, since it seems that it can actually be consistently dodged, but it looks like unintended behaviour to me.
